Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. announces that on December 23, 2015 the Company received an annex to the contract concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services in the scope of outpatient specialist care (cost-consuming outpatient diagnostic services) for the period 1 January to 31 December 2015. Healthcare services referred to hereinabove are provided by St. Anthony’s Hospital in Ząbkowice Śl. owned by EMC Instytut Medyczny S.A. The Issuer informed about the contracts concluded by St. Anthony’s Hospital in Ząbkowice Śl. with the Fund Branch concerning services rendered in the current year (2015) in its current reports nos. 50/2015 of 30.06.2015, 64/2015 of 04.09.2015, 69/2015 of 06.10.2015, 74/2015 of 05.11.2015 and 82/2015 of 27.11.2015 As a result of signing the said annex, the value of the contract with highest value has not changed and it amounts to PLN 12,167,010.00. In connection with concluding the annex referred to hereinabove, the total maximum Fund Branch liability towards the Company for the healthcare services rendered by St. Anthony's Hospital in Ząbkowice Śl. currently totals PLN 14,458,759.34 (an increase by PLN 8,120.34) for the period 1 January to 31 December 2015.
