Report no. 72/2015

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodic information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. announces that on October 29, 2015 the Company received annexes to the contracts concluded by the subsidiary company Powiatowe Centrum Zdrowia sp. z o.o. with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services such as outpatient specialist care, outpatient specialist care within the scope of comprehensive outpatient specialist care over a patient with a chronic disease, psychiatric care and addiction treatment, palliative and hospice care, nursing and care services in terms of long-term care, therapeutic rehabilitation, and inpatient care for the period 1 January to 31 December 2015.

Healthcare services referred to in the said contracts are provided by the subsidiary company of EMC Instytut Medyczny S.A.- Powiatowe Centrum Zdrowia sp. z o.o. in Kowary. The Issuer informed about the contracts concluded by Powiatowe Centrum Zdrowia sp. z o.o. in Kowary with the Fund Branch concerning rendering services in the current year (2015) in its current reports nos. 9/2015 of 20.01.2015, 52/2015 of 1.07.2015, 57/2015 of 28.07.2015, 59/2015 of 10.08.2015 and 61/2015 of 25.08.2015. As a result of signing the abovementioned annexes to the contract with the NHF has changed and it totals PLN 9,410,523.00 (an increase by PLN 69,636.00). As a result of signing the abovementioned annex the total maximum Fund Branch liability towards the subsidiary currently amounts to PLN 17,443,395.88 (an increase by PLN 103,832.55) for the period 1 January to 31 December 2015. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
