Report no. 6/2016

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs that on January 29th, 2016 the Company received from Regionalne Centrum Zdrowia Sp. z o. o. in Lubin a settlement to the contract concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch), regarding healthcare services in the scope of inpatient care for the period 1 January to 31 December 2012. Healthcare services specified hereinabove are provided by the subsidiary of EMC Instytut Medyczny S.A. - Regionalne Centrum Zdrowia Sp. z o. o. in Lubin.

The said company has been the subsidiary of the Issuer since 2014, which the Issuer informed about in its current report no. 24/2014 of 11.02.2014. The above settlement concerns the entire remuneration for providing healthcare services in the scope of inpatient care in the period January 1st 2012 to December 31st 2012 pursuant to a contract that was signed with the Fund Brunch for the period January 1st 2012 to June 30th 2016.The Issuer informed about the contracts for the period January 1st 2014 and December 31st 2015 in its current reports nos. 31/2014 of 18.02.2014, 60/2014 of 10.06.2014, 85/2014 of 23.09.2014, 113/2014 of 29.12.2014, 13/2015 of 2.02.2015, 21/2015 of 13.02.2015, 24/2015 of 18.02.2015, 26/2015 of 18.02.2015, 27/2015 of 19.02.2015, 38/2015 of 25.03.2015, 7/2015 of 16.01.2015, 12/2015 of 30.01.2015, 48/2015 of 10.06.2015, 49/2015 of 16.06.2015, 54/2015 of 8.07.2015, 58/2015 of 7.08.2015, 60/2015 of 21.08.2015, 63/2015 of 3.09.2015, 71/2015 of 16.10.2015, 83/2015 of 30.11.2015, 89/2015 of 07.12.2015, 01/2016 of 4.01.2016.

As a result of signing the said settlement, the value of the contract with the highest value has changed and it currently totals PLN 49,751,708.00 (an increase by PLN 1,222,130.00). The total maximum Fund Branch liability towards the subsidiary Regionalne Centrum Zdrowia sp. z o.o. in Lubin currently amounts to PLN 56,643,432.26 for the period January 1st to December 31st 2012. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital. Pursuant to the settlement, the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) agreed to pay to Regionalne Centrum Zdrowia sp. z o.o additional remuneration in the amount of PLN 1,222,130.00 for healthcare services such as inpatient care provided from 01.01.2012 to 31.12.2012, the number of which exceeded the ones specified in the contract no 03/1/3302673/01/2012/01 for providing healthcare services .
