Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs about receiving on 18 February 2015 annexes to the contracts with the National Health Fund - the Pomorskie Voivodeship Branch in Gdańsk (the Fund Branch)concluded with the subsidiary Zdrowie sp. z o.o. in Kwidzyń regarding healthcare services such as inpatient care, outpatient specialist care, outpatient specialist care, outpatient specialist care (cost-consuming outpatient diagnostic services), primary healthcare in the scope of night and holiday care for the period 1 January to 31 December 2014. Healthcare services referred to hereinabove are provided by the hospital administered by the subsidiary of EMC Instytut Medyczny S.A.- Zdrowie sp. z o.o. in Kwidzyń. The Issuer informed about the contract concluded by Zdrowie sp. z o.o. in Kwidzyń with the Fund Branch concerning healthcare services provided in 2014 in its current reports nos. 19/2014 of 29.01.2014, 89/2014 of 10.10.2014 63/2014 of 02.07.2014, 89/2014 of 10.10.2014 and 105/2014 of 8.12.2014. As a result of signing the abovementioned annexes, the value of the contract with highest value has changed and and it currently amounts to PLN 22,345,104.80 ( an increase by PLN 101,297.48). The total maximum Fund Branch liability towards the Subsidiary with respect to health care services rendered by Zdrowie sp. z o.o. in Kwidzyń currently amounts to PLN 29,001,354.60 (an increase by PLN 102,546.28) for the period 1 January to 31 December 2014. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
