Report no. 12/2015

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. reports that on 30 January 2015 the Company received from Regionalne Centrum Zdrowia Sp. z o. o. in Lubin a contract concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch), regarding healthcare services in the scope of inpatient treatment-prescription drug programs for the period 1 January to 31 December 2015. Healthcare services specified hereinabove are provided by the Hospital administered by the subsidiary of EMC Instytut Medyczny S.A. - Regionalne Centrum Zdrowia Sp. z o. o. in Lubin. The Issuer informed about the contracts concluded by Regionalne Centrum Zdrowia Sp. z o.o. concerning services rendered in 2015 in its current report no. 7/2015 of 16.01.2015. As a result of signing the said contract the value of the contract with the highest value has not changed and it currently amounts to PLN 45,620,918. The total maximum Fund Branch liability towards the subsidiary of the Issuer with respect to medical services provided by the Hospital administered by Regionalne Centrum Zdrowia Sp. z o.o. currently amounts to PLN 53,637,103.15 for the period 1 January to 31 December 2015. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
