Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs about receiving as of 27 January 2015 annexes to the contract concluded with the National Health Fund the Mazowieckie Voivodeship Branch in Warsaw (the Fund Branch) regarding healthcare services in the scope of inpatient treatment for the period 1 January to 31 December 2014. Healthcare services referred to hereinabove are provided by the Non-public Healthcare Centre-St. Anne’s Hospital in Piaseczno managed by the Issuer’s subsidiary company EMC Piaseczno sp. z o. o. The Issuer informed about the contracts with the Fund Branch concerning services rendered in 2014 in its current reports nos. 26/2014 of 13.02.2014, 33/2014 of 21.02.2014, 65/2014 of 09.07.2014, 70/2014 of 23.07.2014, 108/2014 of 17.12.2014 and 8/2015 of 20.01.2015. As a result of signing the abovementioned annex, the value of the contract with the highest value has changed and currently amounts to PLN 20,864,607.70 (an increase by PLN 87,529.30). The total maximum Fund Branch liability towards the Company currently amounts to PLN 23,226,542.70 (an increase by PLN 87,529.30) for the period 1 January to 31 December 2014. A criterion for a contract to be considered significant: The value of contracts exceeds 10% of the Issuer’s equity capital.
