Report no. 80/2014

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. announces that on 21 August 2014 the Company received an annex to the contract concluded with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services in the scope of outpatient specialist care for the period 1 January to 31 December 2014.

Healthcare services referred to hereinabove are provided by St. Anthony’s Hospital in Ząbkowice Śl. owned by EMC Instytut Medyczny S.A.

The Issuer informed about the contracts concluded by St. Anthony’s Hospital in Ząbkowice Śl.  with the Fund Branch concerning services rendered in the current year (2014) in its current reports nos. 10/2014 of 23.01.2014, 56/2014 of 03.06.2014 and 61/2014 of 10.06.2014.

As a result of signing the said annex, the value of the contract with highest value has not changed.

In connection with concluding  the annex referred to hereinabove, the total maximum Fund Branch liability towards  the Company for the healthcare services rendered by St. Anthony's Hospital in Ząbkowice  Śl. currently totals  PLN  13,838,905.00 (an increase by PLN 12,134.80) for the period 1 January to 31 December 2014.

A criterion for a contract to be considered significant:
The value of contracts exceeds 10% of the Issuer’s equity capital.
