Pursuant to §5 (1) (22) of the Regulation of the Minister of Finances of 19 October 2005 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs that Mr. Zdzisław Cepiel was appointed  by the Supervisory Board  as a member of the Management Board at the meeting held on 1 June 2012.

Mr. Zdzisław Cepiel – holds a University degree. In 1972 he obtained a Master of Science degree  in Engineering from Wrocław University of  Technology, the Faculty of Electrical Engineering. In 1998 he completed postgraduate studies at Health Economics School - Faculty of Economic Sciences of the University of Warsaw.

Professional career and public duties currently undertaken :
(1972 – 1975)  Wrocław train depot - Branch Manager;
(1975 – 1982) Railway Ancillary Services Company- Director of RASC;
(1982 – 1984) Department of Electric Power Supply of  Polish State Railways in Wrocław-Section Manager;
(1984 – 1996) Lower Silesian Regional Directorate of National Railways in Wroclaw - Section Manager;
(1996 – 1999) District Railway Hospital in Wroclaw - Deputy Director ( Administration and Technical Research);
(1999 – 2001) Independent Public Clinical Hospital No. 1 in Wrocław- Deputy Director (Administration and Economics);
(2001 – 2003) “EuroMediCare” Instytut Medyczny Sp. z o.o. Specialist Hospital with Outpatient Clinic-Administrative Director;
(2003 – 2007) Lower Silesia Voivodeship Branch of the National Health Fund in Wroclaw - the Head of the Department;
(2007 –  ) EMC Instytut Medyczny S.A – Director for Administration and Finance
(12.2010 – ) Supervisory Board member of EMC Piaseczno Sp. z o.o.
(04.2012 – ) Supervisory Board Member of  Mikulicz Sp. z o.o.

Mr. Zdzisław Cepiel remains employed with the Company.
In accordance with the submitted statement, Mr. Zdzisław Cepiel does not conduct any business activities competitive to EMC Instytut Medyczny S.A. and is not engaged as partner in any partnership competitive to EMC Instytut Medyczny S.A. nor as member of a governing body in any other competitive company or legal entity.  Mr. Zdzisław Cepiel has not been listed in the Register of Insolvent Debtors as maintained under  the Law on the National Court Register.
