Report no. 34/2013

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finances of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A., in connection with receiving as of 18 April  2013:
 

- an annex to the contract concluded by the subsidiary company “Mikulicz” sp. z o.o.  with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services in the scope of in-patient treatment for the period 1 January  to 31 December 2012 and  

- a settlement to the contract concluded by the subsidiary company “Mikulicz” sp. z o.o.  with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) regarding healthcare services in the scope of in-patient treatment for the period 1 January  to 31 December 2010. 

 

Healthcare services referred to hereinabove are provided by the subsidiary company “Mikulicz” Sp. z o.o. in Świebodzice owned by EMC Instytut Medyczny S.A.

 

The Issuer informed about the contracts concluded by Mikulicz sp. z o.o. with the Fund Branch concerning services rendered:
 

- in 2012 in its current report nos. 5/2012 of 20.01.2012, 16/2012 of 30.01.2012, 24/2012 of 15.02.2012, 52/2012 of 31.05.2012, 68/2012 of 02.07.2012, 74/2012 of 08.08.2012, 89/2012 of 05.09.2012, 94/2012 of 07.09.2012 and 29/2013 of 25.03.2013,

- in 2010 in its current reports nos. 49/2010 of 30.06.2010; 71/2010 of 28.09.2010; 12/2011 of 18.02.2011; 39/2011 of 14.03.2011; 43/2011 of 16.03.2011 and 54/2011 of 29.03.2011.

 The Issuer informed about the contracts concerning rendering healthcare services by the said hospital  in 2013 in its current report no 17/2013 of 05.02.2013.

 

As a result of signing the said annex regarding healthcare services in the scope of in-patient treatment rendered in 2012, the value of the contract with highest value has changed and and  it currently amounts to PLN 10,949,310.00 (an increase by PLN 1,447.80).  The total maximum Fund Branch liability towards the subsidiary company “Mikulicz” Sp. z o.o. in Świebodzice currently amounts to PLN 14,266,785.01 (an increase by PLN 1,447.80)for the period from 1 January to 31 December 2012. 
 

 As a result of signing the said annexes and settlement regarding healthcare services in the scope of in-patient treatment rendered in 2010, the value of the contract with highest value has changed and and  it currently amounts to PLN  10,499,227.47 (an increase by PLN 124,736.47).  The total maximum Fund Branch liability towards the subsidiary company “Mikulicz” Sp. z o.o. in Świebodzice currently amounts to PLN 12,985,694.66 (an increase by PLN 124,736.47) for the period from 1 January to 31 December 2010. 
 

The other material terms of the contract with the highest value made public remain unchanged. 

 

A criterion for a contract to be considered significant: 

The value of contracts exceeds 10% of the Issuer’s equity capital.
