Report no. 111/2011

Pursuant to §5 (1) (3) of the Regulation of the Minister of Finance of 19 February 2009 on current and periodical information provided by issuers of securities, the Management Board of EMC Instytut Medyczny S.A. informs about receiving as of 24 November 2011 an  annex to the contract concluded by a subsidiary Company “Lubmed” sp. z o.o. with the National Health Fund - the Dolnośląskie Voivodeship Branch in Wrocław (the Fund Branch) concerning provision of healthcare services in the scope of  outpatient specialist care for the period from 1 July to 31 December 2011.

Healthcare services referred to hereinabove are provided by the subsidiary company “Lubmed” Sp. z o.o. in Lubin owned by EMC Instytut Medyczny S.A.

The Issuer informed about the contracts concluded Lubmed sp. z o.o. with the Fund Branch concerning rendering services in 2011 in its current reports nos. 22/2011 of 01.02.2011 and 42/2011 of 16.03.2011 and 81/2011 of 08.07.2011.

As a result of signing the annex the total maximum Fund Branch liability towards the subsidiary company “Lubmed” sp. z o. o. for the period from 1 January to 31 December 2011 has changed and currently amounts to PLN. 7,322,428.29  (an increase by PLN. 26,450.00).

The other material terms of the contract with the highest value made public remain unchanged.
